No, traffic signal failures do not cause fatal collisions. I observe this whenever I come to an intersection with failed signals. Drivers stop, evaluate the situation, and take turns getting through the affected intersections.

Drivers' default reliance on traffic signals can lead to fatal collisions. Some drivers try to beat a yellow light, or just drive through a red light, crashing into another vehicle proceeding through a green light.

Britain has been removing traffic lights. Most of the intersections they have been removed from delay traffic less than the lights did. Of course that doesn't happen at really busy intersections.
